I would recommend the following treatment options for xanthelasma:

 

1. Phenol application with a toothpick, performed once every 2 weeks.

2. TCA (Trichloroacetic acid) 50-70% application with a toothpick, done once every 2 weeks.

3. Electrodessication.

4. Fractional CO2 laser treatment.

These treatments typically require multiple sessions, with the potency and settings gradually increased based on the patients skin type and tolerability.
