Patient Question

Hello Doctor,

I am reaching out to you regarding a concerning issue I have been facing since my sinus surgery back in November. Following the surgery, my physician conducted a culture, which revealed the presence of Pseudomonas Aeruginosa. Subsequent to this, in February, another culture was performed, uncovering Staphylococcus Aureus and Streptococcus Pneumoniae. To my dismay, the recent culture conducted in April indicated a recurrence of Staphylococcus Aureus and Pseudomonas Aeruginosa. Despite diligently following the prescribed regimen of nasal rinse antibiotics thrice and oral antibiotics twice, I regret to inform you that I have not experienced any alleviation of my symptoms.

Hello,

Thank you for reaching out to Dr. Galen. Please find the below response to your query.

I understand the distress caused by persistent sinus infections despite undergoing treatment. Allow me to provide you with comprehensive guidance tailored to your situation.

Infections of the paranasal sinuses often pose a challenge due to their resistance to conventional medications, largely attributed to the unique nature of the nasal mucosa. To effectively address this issue, it is imperative to adopt a multifaceted approach aimed at reducing the bacterial load within the nasal cavity. This entails meticulous and consistent nasal washing, ensuring a frequency of no less than thrice daily, with each session lasting a minimum of three minutes. Sustaining this regimen for a duration of approximately three months is crucial to observe significant improvement in your condition.

Additionally, incorporating mucolytic pills, preferably slow-release formulations, into your daily routine for an extended period of three months can further aid in promoting sinus drainage and alleviating congestion. Hydration plays a pivotal role in facilitating the body's natural defense mechanisms against infections. Therefore, I strongly advise maintaining adequate fluid intake, aiming for a minimum of three liters per day, unless contraindicated by underlying medical conditions such as heart disease.

It is important to exercise patience and perseverance while adhering to this protocol, as tangible results may take time to manifest. Intermittent courses of antibiotics may be prescribed to complement the aforementioned measures and address acute exacerbations as needed.

Thanks
