I have a small penis (about 3" flaccid) and want to make it much smaller and thinner, ideally inverted. I’d also like my testicles to shrink to about pea size. I want to be permanently unable to get erections. Are there safe, permanent ways to do this?

1. Permanently shrinking the penis and testicles to the extent you describe is not medically supported and carries serious risks. Complete removal of the penis or testicles (penectomy or orchiectomy) is irreversible and typically only done for cancer, trauma, or gender-affirming surgery. These procedures eliminate erections but come with risks like infection, urinary problems, infertility, and require lifelong hormone therapy.
2. Non-surgical methods such as hormone therapy can reduce testicle size somewhat but do not significantly affect penile size. Devices like vacuum pumps do not cause permanent shrinkage. Intentionally damaging erectile tissue to prevent erections can lead to chronic pain and complications.
