Hello Doctor,

 

I've experienced loss of voice, sore throat, ear pain, and vertigo. Strep and COVID tests came back negative. What else could be causing these symptoms? Allergy medicine isn't providing relief either.

Inner Ear Infections: Otitis Media and Interna

 

If you're experiencing vertigo, sore throat, ear pain, and hoarseness or loss of voice, despite negative COVID and streptococcal tests, it's likely due to inner and/or middle ear infections such as otitis media and otitis interna. These infections can result from bacterial infections or barotrauma, which disrupts air pressure within the ear cavity, often due to altitude changes.

 

Other Contributing Factors

 

Risk factors for these conditions include diabetes, hypertension, and disorders like Ménière's disease, characterized by fluid-filled lymphatic canals in the inner ear. Ménière's disease has no known cure, and its cause is often unclear.

 

Treatment and Management

 

For bacterial otitis media or interna, antibiotics are typically prescribed. Resting your voice and avoiding excessive use can help, along with regular intake of warm, humid water. Antibiotic treatment may include parenteral IV co-amoxiclav followed by oral co-amoxiclav. If symptoms persist, consultation with an ENT doctor or otolaryngologist is advisable.
