Patient Question

Hello Doctor,

Before 2 years I had a hemorrhagic cyst and took Yaz, which made me feel better. But recently, mild to moderate fluid has been seen around my right ovary, with approximate measurements of 31 mm × 21 mm × 31 mm and a volume of 12.3cc in the right uterine tube. On Doppler study, mild vascularity was observed around the right ovary. Could you explain what this might mean?

Hello,

Thank you for reaching out to Dr. Galen. Please find the below response to your query.

1. Yes, it is possible that the findings could indicate hydrosalpinx, though it is not the most typical cause of fluid around the ovary. Hydrosalpinx refers to a condition where the fallopian tube becomes blocked and filled with fluid, which can cause swelling and sometimes be associated with mild vascularity. In your case, the description of fluid near the right ovary and the Doppler finding of mild vascularity could potentially point to a hydrosalpinx if the fluid is indeed located in the fallopian tube rather than just around the ovary itself.

2. Typically, hydrosalpinx is identified via imaging such as ultrasound, and it can sometimes be accompanied by pain or changes in fertility. It’s important to differentiate between the types of fluid collections around the ovary (which could be ovarian cysts, hydrosalpinx, or other pelvic fluid collections).

3. I would recommend discussing this with your gynecologist or reproductive specialist, who may suggest additional diagnostic tests (like a hysterosalpingogram or laparoscopy) to confirm the diagnosis and determine if any treatment is needed.

Thanks
