I am a 24-year-old male who has been treated for anxiety disorders for 2 years. I took a break from my medications over the summer (except for a low dose of trazodone for sleep and occasional zopiclone). Recently, my anxiety became difficult to control, and I was prescribed Mirtazapine and Imovane. However, I started to dread going to sleep, leading to nights without sleep—up to 12 days at a time. My cognitive function declined, my eyesight worsened, and I experienced gaps in my memory.
I finally visited a psychiatrist in November and was prescribed quetiapine and pregabalin, but my fear of sleep has become overwhelming. I struggle with trust issues and find it hard to engage in daily activities. I've been irrationally afraid of medications and live with my parents, using my sleep issues to prove my mental struggles. My doctor referred me to a psychiatric hospital but labeled my condition as F13, which I don't believe fits.
Should I go to the hospital, or seek another psychiatric appointment for a proper diagnosis? Is my memory loss due to sleep deprivation, prolonged zopiclone use, or both?

Firstly, I think the diagnosis of F13 is not the best fit for you. You have provided a lot of information about your symptoms, which is very helpful in determining if you’re experiencing symptoms from a primary mental disorder, rather than just general symptoms that may arise from medication misuse. I must admit that the prescription you are currently taking may not effectively address your symptoms. You are on subtherapeutic doses of all your medications, and it’s important to have an adequate treatment plan to see real improvements.
Currently, Mirtazapine may be used for sleep induction but may not be the most suitable option for your anxiety and related symptoms. Additionally, I suspect that you are experiencing racing thoughts, difficulty controlling your feelings, mood swings, poor concentration, and lack of focus.
I truly believe that your memory issues are a consequence of the psychiatric symptoms you are experiencing. I strongly recommend seeking further evaluation from a psychiatrist who can provide a more accurate diagnosis and develop a suitable treatment plan tailored to your needs.
