I'm 47 years old and haven't had a period since I was 42. But now, I've been bleeding heavily for the past month, and I'm passing large clots. I'm not sure if this is normal or something serious. Should I be worried?

1. It sounds like you may have gone through menopause around 42, and now experiencing heavy bleeding with clots after five years could indicate several possible causes. This could be due to hormonal imbalances, endometrial hyperplasia (thickening of the uterine lining), fibroids, polyps, infections, or even more serious concerns like endometrial cancer. Conditions like hypertension could also contribute to abnormal bleeding.

 

2. To determine the exact cause, you will need a thorough evaluation. This may include a Pap smear, an ultrasound of the pelvis, and an endometrial biopsy (using a Pipelle or hysteroscopic D&C) to check the uterine lining. Blood tests will also be done to assess for anemia due to prolonged bleeding.

 

3. Based on the findings, a specialist gynecologist will suggest the best treatment. If the bleeding is due to hormonal imbalance, medication may help control it. However, if there is an underlying issue that requires surgery, your doctor will discuss the options.
