Patient Question

Hello Doctor,

Is it true that a male alter in a female body is male within the brain? Do they have brain activity similar to someone with a male identity in Dissociative Identity Disorder?

Hello,

Thank you for reaching out to Dr.Galen. Please find the below response to your query.

In Dissociative Identity Disorder (DID), the concept of alters—including male alters in a female body—is complex and often misunderstood. Alters, or distinct identities, can possess different gender identities, preferences, behaviors, and emotional responses. However, all alters are part of the same brain and nervous system, which fundamentally remains unchanged between different identities.

Regarding brain activity:

  • Brain Imaging Studies: Some neuroimaging studies suggest that different alters can exhibit unique patterns of brain activity. These changes might relate to emotional states, sensory processing, or cognitive functions. However, this does not imply that a male alter has the same brain activity as a biological male; instead, the brain may show varied activation patterns linked to the alter's identity, regardless of physical sex.

  • Gender Identity: A male alter in a female body may express thoughts, emotions, and behaviors that align with a male gender identity, potentially influencing neural circuits related to gender identity and self-perception. Nevertheless, this occurs within the same brain structure, shaped by the individual’s experiences, memories, and psychological processes.

In summary, while alters may have distinct experiences, there is no evidence that a male alter would have identical brain activity to that of a cisgender male. The brain reflects the alter’s identity, but it remains part of the same physical body.

Thanks
